The psychological drama revolves around Jessica and Karl, a loving couple with their daughter Rhiannon. After years of trying to expand their family, they are thrilled to adopt Gabby, an 8-year-old orphan with some personal challenges. 

However, their dream of a perfect family starts to crumble when they realize Gabby is a troubled child. As Gabby's disturbing behavior escalates and secrets come to light, the family's harmony is shattered. Jessica grows increasingly worried that Gabby might be a threat to their family.

The movie is loosely premised on the adoption saga of Natalia Grace. In 2010, Michael and Kristine Barnett adopted Natalia, believing she was a 6-year-old girl. However, they soon grew suspicious of her age, claiming she was actually an adult woman with dwarfism (  a condition known as spondyloepiphyseal), posing as a child with harmful intentions.

The Barnetts alleged that Natalia tried to hurt them and their biological children. They claimed she attempted to electrocute Kristine, poison her coffee, and placed thumbtacks on the stairs. In 2012, two years after adopting Natalia, they successfully petitioned Marion County Probate Court to change her age to 22, altering her birth year from 2003 to 1989.

The following year, the Barnetts moved to Canada with their three sons, leaving Natalia alone in an apartment in Lafayette. After their divorce, Michael and Kristine were charged with neglect of a dependent.
